Charge Nurse – Supported Discharge Unit (Band 6)

About the role

The Supported Discharge Unit at NHS Fife is seeking a Band 6 Charge Nurse to provide leadership and direction across the unit and related areas. The role supports safe patient discharge, works closely with the Discharge HUB and Social Work, and contributes to maintaining site capacity and flow.

Key responsibilities

• Lead and support multidisciplinary team (MDT) meetings to coordinate discharge pathways.
• Provide charge cover for the Nursing Pool, Discharge Lounge, Ward 9 and an additional Supported Discharge Unit.
• Collaborate with the Senior Charge Nurse and Clinical Nurse Manager to ensure high‑quality patient‑centred care.
• Monitor and promote effective discharge planning, including setting Planned Date of Discharge (PDD).
• Drive change initiatives and support continuous improvement within the unit.

Required profile

• Post‑registration nursing experience, preferably in discharge or acute care settings.
• Demonstrated ability to lead, manage and develop teams.
• Strong interpersonal and communication skills for working with MDT members and external partners.
• Experience of change management and delivering high standards of care.
• Willingness to become a member of the Protection of Vulnerable Groups (PVG) Scheme.
