Advanced Practitioner – ANP Team (Annex 21)

About the role

The Advanced Practitioner will work autonomously across primary, community and acute services, providing clinical decision‑making for patients 24/7, including public holidays. This is an Annex 21 training post that leads to a Band 7 position upon successful completion.

Key responsibilities

• Assess, diagnose and treat acute, non‑acute and chronic conditions across the lifespan.
• Provide clinical care and management for stable and acutely unwell patients and their families.
• Communicate daily with the multidisciplinary team, OOH hub and acute services to ensure continuity of care.
• Deliver 12‑hour shifts over a 7‑day working week, covering out‑of‑hours periods.
• Participate in research collaborations with academic and multidisciplinary partners.

Required profile

• Registered Nurse, Allied Health Professional or Pharmacist with valid NMC/HCPC/GPHC registration.
• Educated to or working towards a Masters level.
• Post‑graduate diploma in Advanced Clinical Practice and specialist expertise relevant to the role.
