Clinical Support Worker – Discharge Lounge

About the role

The Clinical Support Worker will be based in the Discharge Lounge of NHS Lanarkshire hospitals, providing essential care and coordination for patients awaiting discharge. You will work closely with nurses, transport services, social work, pharmacy and other multidisciplinary teams to ensure a smooth and safe transition home.

Key responsibilities

• Maintain a high standard of nursing care for patients from all specialties while they wait in the Discharge Lounge.
• Liaise with transport, social work and pharmacy to organise timely discharges.
• Coordinate between internal professional groups and external care agencies.
• Track and collect prescriptions regularly.
• Assist with bed‑busting, flow team activities and ordering/reviewing of beds, mattresses and commodes.
• Collect routine urine and blood specimens and record patient vital signs.
• Provide escort duties, housekeeping support and equipment checks (e.g., blood glucose monitors).
• Prepare equipment and procedure trolleys according to manuals.
• Document patient observations, changes in condition and maintain accurate records.

Required profile

• SVQ Level III in Healthcare or equivalent experience in a caring environment.
• Ability to work independently and as part of a multidisciplinary team.
• Strong written and verbal communication skills.
• Capability to perform patient‑care tasks efficiently in a busy setting.
