Staff Nurse – Supported Discharge Unit

About the role

Victoria Hospital is seeking enthusiastic registered nurses to join the Supported Discharge Unit. Two posts are available – one part‑time (28.8 hours/week) and one full‑time (36 hours/week) – working in a fast‑paced, nurse‑ and doctor‑led environment that bridges acute care and community settings.

Key responsibilities

• Collaborate with the multidisciplinary team, including physiotherapists, occupational therapists, social workers and the Health and Social Care Partnership.
• Coordinate safe and patient‑centred discharges, ensuring individual needs are met.
• Facilitate transfers of care to other environments and support patients returning home.
• Contribute to the development of care pathways and quality improvement initiatives.
• Deliver safe, effective care that enhances the patient experience and aligns with organisational objectives.

Required profile

• Registered Nurse with a commitment to patient‑centred care.
• Excellent communication and teamwork skills.
• Desirable post‑registration experience and ability to demonstrate clinical and professional development.
• Understanding of acute medical and chronic illness complexities.
• Willingness to work a rotational schedule including days, nights and weekends.
