Advanced Frailty Practitioner

About the role

The Acute Frailty team at Victoria Hospital is seeking an Advanced Frailty Practitioner to enhance care for patients living with frailty in the acute hospital setting. This role involves early identification, comprehensive assessment, and ongoing medical review across front‑door areas and the Acute Frailty Unit.

Key responsibilities

• Conduct rapid, multidimensional frailty assessments in Emergency, Admissions, and GP Assessment units.
• Develop and implement individualized care pathways and coordinate timely discharge planning.
• Provide daily medical review of frail patients alongside a multidisciplinary team.
• Collaborate with health and social care partners to ensure integrated pathways.
• Contribute to the development and expansion of the Acute Frailty Unit.

Required profile

• Demonstrated experience managing patients with frailty in an acute setting.
• Strong organisational and communication abilities, with empathy for patients and carers.
• Commitment to continuous professional development and collaborative working.
• Willingness to join the Protection of Vulnerable Groups (PVG) Scheme.

Required skills

• Knowledge of frailty syndromes and assessment tools.
• Experience in acute hospital care and multidisciplinary team environments.
