Complex Discharge Planner (Remote)

About the role

Centene is seeking a Remote Complex Discharge Planner to develop and coordinate comprehensive discharge plans for high‑risk members. The role ensures a smooth transition from acute care to the appropriate level of care while educating members and caregivers.

Key responsibilities

• Develop complex discharge plans based on thorough needs assessments.
• Coordinate with providers, community agencies, and utilization review staff to secure safe, effective transitions.
• Maintain accurate documentation of discharge plans in health management systems.
• Educate members and families on discharge processes and utilization guidelines.
• Provide feedback to leadership for continuous improvement of discharge planning.
• Assist with prior authorizations and support concurrent review teams as needed.

Required profile

• Graduate of an accredited nursing program (RN) with an active Compact RN license.
• 2–4 years of related experience, including at least 2 years in acute care.
• Experience in case management or discharge planning, preferably in a telephonic environment.
• Knowledge of Medicare, Medicaid, and utilization management regulations is preferred.

Required skills

• RN licensure (Compact State licensure acceptable).
• Case management and discharge planning competencies.
• Acute‑care clinical experience.
• Familiarity with Medicare/Medicaid regulations and utilization management processes.
