Advanced Practitioner – ANP Team (Annex 21)

About the role
The Advanced Practitioner will deliver autonomous clinical care across primary, community and acute services for NHS Lanarkshire, covering both in‑hours and out‑of‑hours periods, including weekends and public holidays. The role operates on a 12‑hour shift pattern over a seven‑day week.
Key responsibilities
- Assess, diagnose and treat patients with acute, non‑acute and chronic conditions across the lifespan.
- Make independent clinical decisions and manage care pathways for stable and acutely unwell patients.
- Communicate daily with the multidisciplinary team, OOH hub and acute services to ensure seamless handover.
- Utilise medical telehealth/telephone support when required.
- Manage time effectively to balance clinical, educational and administrative duties.
- Participate in research collaborations with academic partners and higher education institutions.
Required profile
- Registered Nurse, Allied Health Professional or Pharmacist with valid NMC/HCPC/GPHC registration.
- Educated to or working towards Masters level.
- Post‑graduate diploma in Advanced Clinical Practice and specialist expertise relevant to the role.
