Heart Failure Clinical Nurse Specialist - Cardiology

About the role

The Heart Failure Clinical Nurse Specialist will join NHS Greater Glasgow and Clyde’s Interface Care Division to lead innovative, nurse‑led ambulatory and virtual services for heart failure patients. Working from Queen Elizabeth University Hospital, you will help transform care pathways, bringing treatment closer to home and improving patient outcomes.

Key responsibilities

• Provide clinical leadership to the Heart Failure service, assessing patient needs and delivering evidence‑based programmes of care.
• Deliver nurse‑led ambulatory care, virtual clinics and remote monitoring for heart failure patients.
• Assess clinical status using examination and history‑taking to develop individualised pharmacological and non‑pharmacological management plans.
• Support patients across the care pathway, including early supported discharge, in‑reach services and liaison at the front door and on the ward.
• Maintain and develop own clinical skills to meet evolving service requirements.

Required profile

• Registered Nurse with specialist experience in heart failure and cardiology.
• Demonstrated ability to work within multidisciplinary teams and lead service development.
• Experience with ambulatory care, virtual clinics or remote monitoring is essential.
• Commitment to evidence‑based practice and continuous quality improvement.

Required skills

• Clinical assessment and history‑taking for cardiac patients.
• Development and implementation of evidence‑based care pathways.
• Use of electronic triage and virtual care platforms.
