Heart Failure Clinical Nurse Specialist – Cardiology

About the role

The Interface Care Division at NHS Greater Glasgow and Clyde is seeking a Heart Failure Clinical Nurse Specialist to lead innovative, nurse‑led ambulatory and virtual services for heart failure patients. This permanent, full‑time position is based at Queen Elizabeth University Hospital and will play a pivotal role in transforming care pathways across cardiology and heart failure services.

Key responsibilities

• Provide clinical leadership for the Heart Failure service, assessing patient needs and delivering evidence‑based programmes.
• Maintain and develop specialist clinical skills to meet evolving service demands.
• Conduct comprehensive clinical assessments and create individualized pharmacological and non‑pharmacological management plans.
• Plan and initiate complex heart‑failure management, including remote monitoring and virtual clinic delivery.
• Collaborate with multidisciplinary teams to support ambulatory care, early supported discharge, in‑reach services, and patient liaison at front‑door and ward levels.

Required profile

• Registered Nurse with specialist experience in heart failure patient care.
• Proven ability to work across cardiology, ambulatory, and virtual care settings.
• Strong understanding of evidence‑based heart‑failure management and patient‑centred care.
