Senior Clinical Pharmacist – Frailty

About the role

Join NHS Fife’s Frailty & Community Health Pharmacy Service as a Senior Clinical Pharmacist. You will work within a multidisciplinary team to optimise medication for patients with complex polypharmacy, supporting safe prescribing and treatment outcomes across Fife.

Key responsibilities

• Provide specialist clinical pharmacy support for frail patients receiving multiple medicines.
• Collaborate with consultants, specialist nurses and the wider MDT to develop and implement frailty service pathways.
• Lead medication reviews, optimise therapy, and monitor patient outcomes.
• Contribute to clinical governance, quality improvement and education initiatives.
• Support service redesign and transformation programmes within the Frailty & Community Health team.

Required profile

• Qualified pharmacist with extensive experience in clinical pharmacy.
• Demonstrated ability to work with complex polypharmacy regimens.
• Strong communication skills for conveying clinical information to patients and professionals.
• Leadership experience in multidisciplinary settings.
• Independent Prescribing qualification is highly desirable.
