Senior Clinical Pharmacist – Frailty (LW)

About the role

Join NHS Fife’s Frailty & Community Health Pharmacy Service as a Senior Clinical Pharmacist. You will work within a multidisciplinary team to deliver high‑quality, patient‑centred care for older adults with complex polypharmacy needs.

Key responsibilities

• Provide expert medication review, safe prescribing and treatment optimisation for frail patients.
• Support the development and redesign of the frailty pharmacy service.
• Collaborate with consultants, specialist nurses and other MDT members to make joint clinical decisions.
• Contribute to clinical governance, quality improvement, education and medicines optimisation at service and organisational level.
• Lead service innovation and help shape the strategic direction of pharmacy services within NHS Fife.

Required profile

• Registered pharmacist with extensive experience in clinical pharmacy.
• Demonstrated ability to work with complex polypharmacy regimens.
• Strong collaborative leadership and communication skills.
• Commitment to patient‑centred care and continuous service improvement.

Required skills

• Independent Prescribing qualification (highly desirable).
